According to the American Society of Echocardiography (ASE) and the ESC/EACTS, severe mitral regurgitation (MR) is defined by the presence of at least four of the following echocardiographic parameters:
- Flail leaflet
- Vena contracta width (VCW) of at least 0.7 cm
- PISA radius of at least 1.0 cm
- Central large color Doppler jet that is more than 50% of the left atrium (LA) area
- Pulmonary vein systolic reversal
- Enlarged left ventricle (LV) with normal function
- Elliptical-shaped orifice
Other echocardiographic criteria for severe MR include:
- A large coaptation defect
- A very large color flow central jet or eccentric jet that swirls, adheres, and reaches the LA’s posterior wall
- A dense or triangular continuous-wave signal of regurgitant jet
- A large flow convergence zone
