According to the American Society of Echocardiography (ASE) and the ESC/EACTS, severe mitral regurgitation (MR) is defined by the presence of at least four of the following echocardiographic parameters:

  • Flail leaflet
  • Vena contracta width (VCW) of at least 0.7 cm
  • PISA radius of at least 1.0 cm
  • Central large color Doppler jet that is more than 50% of the left atrium (LA) area
  • Pulmonary vein systolic reversal
  • Enlarged left ventricle (LV) with normal function
  • Elliptical-shaped orifice 

Other echocardiographic criteria for severe MR include:

  • A large coaptation defect
  • A very large color flow central jet or eccentric jet that swirls, adheres, and reaches the LA’s posterior wall
  • A dense or triangular continuous-wave signal of regurgitant jet
  • A large flow convergence zone
